For homes and households. Have ready:

Your Social Security number and date of birth
Your employer and income
Your bank and credit references
Your spouse’s information, only if your spouse will use the account or sign the agreement

For companies, partnerships, sole proprietors and tax-exempt agencies. Have ready:

Your business name, FEIN and addresses
The name, title and Social Security number of each owner, partner or officer
Two bank references and three trade references
If you are tax-exempt, a copy of your Federal 637 certificate or 501(c)(3) letter. We cannot process the application without it.

Email is not a secure way to send a Social Security number. To keep it private, mail the form (better) or bring it to the office (best).
